• $40.00 Adjustment
• $30.00 Erchonia EVRL Treatment (class IV cold laser)

Dr. Stromberg is a participating provider for the following insurance plans:

• Blue Cross Blue Shield
• Medicare

**All coverage is subject to copays and deductibles stated in the patient's insurance contract.

All others insurance providers will be billed for the patient but reimbursement will depend on out of network coverage.
